Smallholding Visits Forms Pack
The Smallholding Visits Forms Pack contains 6 document templates. The pack is designed for customers that keep small numbers of livestock as pets on their property, or nearby.
The Smallholder Template Pack contains 6 forms to help you take care of a mix of livestock living either around the home or in a separate location.
There is a Meeting Checklist to help you ask the right questions at the initial Meet & Greet, and a Registration Form and Service Agreement template to leave with the customer after you have met with them and you both want to go ahead with a booking. There is a Visit Record to diarise your visits and leave notes for the customer at the end of the booking, as well as a Medicine Log to record any medicines given and issues with giving it. Once registered, there is a Repeat Booking Form for booking again that is a bit shorter.
Please note that this pack is designed for caring for animals considered as pets that have are not part of commercial activities such as milking and slaughter.
The Registration Form, Medicine Log, Repeat Booking Form, Visit Record and Meeting Checklist are 90% complete and just need your company details and logo added before use. The Service Agreement is 75% completed and requires your business rules for subjects such payment and cancellation to be added before use.
If you are uncertain about completing the Service Agreement in a fair and reasonable manner, you can always contact me to discuss what would be a good policy before completing it.
The templates have been created in Microsoft Word and are fully amendable. You must have access to Microsoft Word or Microsoft 365 to make the required changes to your templates. Documents can also be amended in Google Docs with some additional adjustments to the table formats.
Alternatively I can make the changes for you and send you PDF copies of your finished documents instead.
