Dog Grooming Forms Pack

The Groomer Forms Template Pack contains a Customer  Registration Form, a Price List template, and a Groomer Report Sheet.  The fonts and colours can be changed as you wish.  

The Customer Registration Form requires a little bit of completion so that it covers your company's terms and conditions, but the areas that need information are clearly marked.  The vet release is built into the document rather than having things on separate forms; customers no longer have time for lots of forms, and this approach suits them better.  

The Groomer Report Sheet is a good place to make notes about the groom and the dog, so that you can refer back at their next appointment.  The Price List helps you to keep customers informed about current rates.  If you require this as A5 please contact me and I will help you convert it for printing. 

In addition to the Groomer Template Pack, there is also a Risk Assessment Template available for grooming premises, and another that is specific for grooming staff and workers.  These are available separately to the Groomer Template Pack.  Feel free to contact me for details. 

The templates have been created in Microsoft Word and are fully amendable.   You must have access to Microsoft Word or Microsoft 365 to make the required changes to your templates. Documents can also be amended in Google Docs with some additional adjustments to the table formats.

Alternatively I can make the changes for you and send you PDF copies of your finished documents instead.
